## Narrative

He had put bolt in hole and was putting bolt onto wrench. Then stated that coat sleeve had got caught between the bolt and wrench, he began to spin the head to align the bolt into wrench, sleeve became caught between the 2. it caused his right arm to twist around the bolt wrench. The ee continued to work until 4/27/11, he then began missing work due to injury.
