Narrative

An unintentional fall of the roof was discovered on the 1 Right Tail section panel in the # 3 entry at spad # 90 cross cut #42. The fall was approximately 18ft. x 18ft. and 7 feet high. This was in the bleeder entry and will not be cleaned up.
