Employee had reported carpal tunnel like symptoms in his wrist in Feb. 2011. He also reported at this time -- complaining of complications with a hernia surgery from 5 yrs earlier. On 6/17/11- employee had carpal tunnel surgery & had his hernia repaired. His first shift missed was on 6/20/11.
