## Narrative

Mechanic was removing bolts from a hammer mill door utilizing a 3/4"" impact wrench fitted with a 3/4"" to 1/2"" adapter to accept the smaller socket.  The adapter reportedly failed causing the socket to spin off the bolt head, striking the employee in the mouth.
