On 2/23/2011 EE reported having continued pain with his left shoulder. EE sought medical treatment an MRI on 5/28/2011 revealed the following, left shoulder impingement, left AC joint pain/OA, and left bicep tendonitis. Dr. note statement ""I believe with a certain degree of medical certainty that the patients work activities represent a contributing factor"".
