## Narrative

Employee was using a scaling bar to pry a piece of angle iron straight, when the bar slipped he fell forward landing on his hands & knees. He did not see our medic on site. He worked his shift. The next AM (7/10/11)he woke up at home & said he is sore. He was going to see a doctor, not come into work. X-rays resulted in fracture to L. thumb & elbow, sprained R. wrist.
