## Narrative

While taking drill steel out of roof bacon strap, it became separated from the first inby steel in the roof. When it came out of the hole and swung around striking him in the left ear and head. Lacerating ear and head. PPE was not a factor, compliance rules/regs were not a factor, mining equipment/systems was a factor, miner proficiency was a factor, also.
