Narrative

Prying a loose rock down from mine roof, rock pushed back on bar and pinched his right hand between bar and steel beam. Lacerated hand between index finger and thumb. PPE was not a factor, compliance rules/regs were not a factor, mining equipment/systems was not a factor, miner profiency was a factor.
