## Narrative

Employee worked 7/14/11 & called in on 7/15/11 @ 2:40pm stating that his R.eye was bothering him after working.He thinks dust got into his eye & it was watering alot.He went to doctor, no rx issued,but doctor told him to take a few days rest.He did not report this until he called in the next day after seeing the doctor then he went on days off.(2 days were missed)
