Narrative

Employee was making a splice on the 6D belt. While lifting a piece of 48"" top structure, employee felt a pain in the groin area. Employee immediately visited Logan General where he was treated and released with no restrictions. Employee continued to work regular duties up until 7/14/11. Employee had surgery on 7/14/11 to repair a hernia that possibly resulted on 3/31/11.
