Narrative

Employee was using a 4"" hand grinder on a 2"" tubing clamped in a bench vice. While doing so, a small piece of metal got in his right eye. He tried to wash it out, but was unable to remove it. A doctor had to remove the small piece of metal. NOTE: He did have safety glasses on.
