A roof fall measuring 70'L x 19'W x 8'thick was discovered during an exam in the SE Mains along #5 belt, No. 3 entry, inby survey spad 66639. The area was developed 4/5/2011 with 60"" torque tension roof bolts as the primary support. The fall occurred due to laminated coal and rock in the roof. Ventilation and the movement of machines were not affected. No injuries occurred.
