Narrative

On Fri 5/27/11 an EE rec'd an electrical shock. He was seen at hospital, held for observation, & released. He was seen by a panel physician as soon as possible-weekend & holiday made that day 5/31/11. He was released for full duty. This report is being submitted to terminate a citation issued by MSHA stating the Co. didn't submit a 7000-1. These circumstances don't meet Part 50.
