Narrative

Employee starting to bolt an open 20 foot cut in which roof rock had been dislodged and fallen out. After setting the ATRS and while swinging out his safety post canopy, the roof began chipping and the injured was struck by a piece of falling roof rock estimated to be 3.5' long x 2' wide x 10"" thick. The rock caused him to fall striking his right shoulder and ribs on the bottom.
