Employee was gouging clad plate on the morning of August 3, 2011 at the 3 Oaks Mine. While gouging the plate a piece of the gouging rod end broke off and landed on the cuff of his jeans. By the time he had raised his welding hood to find the ""Red Hot"" carbon rod, the rod had burnt through his pants leg and made its way into his boot.
