Narrative

Employee was hauling supplies. Piece of draw rock fell from top, striking employee on back of neck and shoulder. Rock measured 36"" long, 6"" to 12"" wide and 1"" to 4"" in thickness. Employee complained of burning sensation in left arm. Rock fell between 2 bolts. Bolts were measured to be 38"" apart in this location.
