## Narrative

A contractor was making modifications to the exhaust ventilation pipe on the 59-3 GOB ventilation borehole with the 9 stage methane exhauster pump operating. The modifications caused methane to be passed over a hot surface causing methane to ignite which resulted in burns to an employee servicing the methane exhauster.
