Narrative

Employee was using a 1/2"" drive air impact wrench with a swivel socket extension to loosen bolts on a wash screen to change screen cloth. Employee failed to wear provided gloves and grabbed the swivel with his right hand and twisted it while applying air to the impact gun. The swivel caused a 3 cm laceration requiring 9 stitches to close
