Narrative

When changing out 45ft 22in pipe being lifted into place with a 60 ton crane. The injured employee pulled the tag line to position pipe the pipe swung past the attachment point. The injured grabbed the pipe with his left hand to stop the pipe and his left index finger was pinched between the pipe and the hand rail.
