Narrative

Employee was installing a roof bolt into the mine roof. As he applied boom pressure pushing the bolt into the roof with a one (1) foot wrench, the wrench bent. As he begin to rotate the bolt, the wrench came out of the chuck, striking him on the forehead, causing a laceration.
