Narrative

The IE and the hauler operators were attempting to extend the ventilation tube slider, when the roof collapsed. The falling roof hit the tubing which knocked the IE down and then the rock hit the EE on the left foot. The falling material cause laceration to back and laceration and crushing fractures to his lt. foot. Rock size 1' - 3' thick and approx. 20' L. X 8' W.
