Narrative

EE was lubricating a tail pulley and reported slipping in mud created by recent rain storms and twisting his left knee. EE reported the incident to his supervisor the following day 9/29/11 as a first aid only and did not request medical attention until 10/05/11. EE was diagnosed with a ""strain"" to the left knee and released to work with restrictions.
