Narrative

Employee was unhooking a fertilizer buggy from a truck. He was using the jack welded to the frame of the buggy to jack it up. After jacking it up he pulled the pin to unhook the buggy from the truck causing the buggy to shift. The shift of the buggy caused the weld to break on the jack that connected it to the buggy frame causing it to smash employee's foot.
