Narrative

A roof fall occurred in a 4-way intersection at spad no. 11278 measuring 60' x 20' x 12' in thickness. The area was bolted with 3/4"" x 6' tension roof bolts. At least 8 - 10' long cable bolts were installed in the area also. No injuries occurred.
