(D-30A) A ROOF FALL was discovered in the #5 entry at survey station 63-68. The fall area was approximately 120' in length, entry wide and 7-8' in thickness. The area was bolted with 42"" conventional type bolts. Timbers were set to prevent access and Heintzman roof support jacks were set to protect the #4 entry.
