Narrative

Employee was roof bolting in the 8th right section installing belt hangers in the mine roof. Employee was trying to recover a belt hanger from a bent roof bolt when the hanger fell of the bolt striking the employee in the mouth. The impact to the employees mouth loosened his front teeth.
