Narrative

The move crew was outby the section loading belt structure into a scoop bucket. EE bent over to pick up a top 48"" piece of belt structure, when he felt a sharp pain in his groin area. The incident was not reported to mine management until a month later when EE took off to have hernia surgery. Employees working with him confirmed the incident. 1st day away was 10/13/11
