## Narrative

Employee alleges after adjusting jaw crusher tension rods he backed up 8 feet and started up jaw.  The bolt that attaches the tension bar yoke to the moveable jaw failed and the spring pressure caused the rod to release, become airborne and struck him in the left shoulder and left ear.
