## Narrative

A roof fall occurred in the intake air course of the #2 entry of the mains section at survey station #1156. The roof fall did not affect the ventilation or the passage of miners. The fall measured 30 x 65 x 5' 6"" & the area was bolted with fully grouted rods. The fall is a result of the stress applied to weak planes in the thin bedded laminated shale strata over a period of time.
