Narrative

Employee was lifting supplies (T-3 channels) from a fork lift to the roof bolter. As he lifted from a squatting postion, he felt pain in his groin area. He continued to work and during the next few weeks his pain increased. His injuries resulted in a hernia. He started missing time on 11/08/2011.
