Narrative

The employee was climbing from the feed chute when he grabbed the air hose attached to a 3/4"" impact gun the impact gun fell on his left index finger causing bleeding and a Distal Phalange fracture on the left index finger. The employee did not seek medical Treatment until 11/10/11 where the finger was diagnosis as a distal phalange fracture
