Narrative

Employee was aligning belt roller with a hammer, when he struck his left hand against a timber in the belt entry. Employee reported this incident on 11/9, sought medical treatment on 11/11. Due to a previous un-work related injury to his hand it was not determined to be work related until 11/18/2011
