## Narrative

After scaling the roof in the #2 to #3 x-cut 1 block outby the 002 section tailpiece, the roof bolter operator trammed the bolter forward to spot bolt an already bolted area. He set up the ATRS, crawled under the operator side canopy and then a rock fell striking him on the back, neck, & shoulders.
