## Narrative

Employee was struck on the upper right arm, right leg, and right foot and ankle by a rock that fell out between roof bolts. The rock measured 80in. x 42in. x 0-10in. He received fractures to h is upper right arm and upper right leg. His right foot and ankle had to be surgically amputated 6 in. above the ankle due to the crushing injuries they received.
