Narrative

Adjusting conveyor belt with 3/4"" ratchet it was tight and pulled hard. Employee made a note on his time-card but did not tell anyone. Time-cards submitted days later. Sup. asked employee about note on T/C but employee indicated all was fine only that his shoulders hurt. Did not work 11/30/11 then came in 12/1/11 saying he saw a Dr. and was off work until he could have an MRI.
