## Narrative

EE was setting up to grade structure on the main west belt. EE was hanging a come along while standing on a ladder when he slipped and fell off. He landed face first, and injured his lower back. The ladder was set on level ground floor to top approx. 9'. Factor contributing to incident was inattention and improper method.
