## Narrative

Worker was attached to a 30' tall rebar cage @ the 10'-15' (foot) height. For an undetermined reason the cage collapsed, bringing worker down with it. A rebar struck him over the left eye and across the ribs. The blow resulted in swelling over the left eye & soreness of the chest & ribs. Required medical attention & several days off to recover.
