## Narrative

Co-worker was holding tension on the reject colander shaft with a pipe wrench so injured employee could add two links to chain on reducer.  while injured emp was attempting to install master link lock, the pipe wrench slipped causing it to catch his left hand middle finger in a pinch point between chain and sprocket result in laceration requiring stitches.
