## Narrative

Employee was driving haul truck, and he began the dumping process. When the bed reached 4' raise, the hoist cylinder failed causing the bed to lower abruptly. This caused a jarring motion to the employee. He felt no pain at the time, but later began to have stiffness in the upper back and neck. Resulted in neck strain and thoracic strain.
