Narrative

Foreman was measuring the tip height on No. 132 shield, when a piece of roof coal fell between the shield tip and roof bolt approximately six feet long, 3 feet wide and 13 inches thick. It struck his foot on the back side causing fracture. Compliance rules/regs not a factor, PPE was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.
