Narrative

Employee states he has pain in his shoulder which he feels is the result of ten-plus years of work-related heavy lifting. Employee was sent for medical evaluation and it was determined he met the Colorado Workers Comp Guidelines for occupational illness. Employee underwent shoulder surgery on 1/4/2012.
