Narrative

Individual was walking along the kulntown rockdust chute when the left foot became caught in a tie down rope causing the individual to trip. The individual fell with the right hand on rail and the left hand on the ground. Individual went off work for surgery to the shoulder - rotator cuff 1/5/12.
