## Narrative

On 12/14/11, the employee stated he was tramming the 550 scoop in the inby direction heading towards the #4 face. He stated he went through a set of fly pads and ran the scoop into the rib causing the scoop to jar and striking his right shoulder and elbow inside of the deck. The incident was deemed a lost time reportable on 1/11/12 for a strain to the shoulder and neck.
