Narrative

Employee was removing drill chuck from drill pot of roof bolter, while using a hammer to remove chuck a piece of metal flew off of the hammer or chuck lacerating his right leg. PPE was not a factor, rules/regs was not a factor, miner knowledge could be a factor, mining/systems could be a factor.
