Employee was assisting roof bolter operators conduct a methane test by the use of an extendable probe when a piece of mine roof fell from between 2 steel channels,(that were supported with 60"" fully grouted rods), striking his right foot, causing a crushing injury to the tip of his big and third toe. The piece of mine roof did not appear to be loose on initial examination.
