## Narrative

Employee was in the process of jacking up a belt tail using a track jack. He felt pain in his right shoulder. He continued to work until 01/20/2012 when he was seen by a physician. An MRI revealed a torn right rotator cuff and a partial tear to his right bicep tendon. Surgery has not yet been scheduled.
