## Narrative

Employee was in a loader/mucker carrying a load rock. As he was moving with a loaded bucket, he ran over a bump in the road causing him to hit his head on the top of the cab of the mucker. Initial medical evaluation cleared the employee without restrictions. Employee notified employer on 02/07/2012 that he saw his physician for continued symptoms resulting in lost time.
