on 2/3/2012, employee reported that he had symptoms of carpal tunnel coming over the last few years. On 2/17/2012- he did have carpal tunnel surgery on his Right hand/wrist. there was no accident. no incident. This is an Occ. Illness - Occ.Illness/LT 2/18/2012 was his 1st shift missed.
