Narrative

Employee was roof bolting on Unit #3 when a rock fell out and stuck him on the front of his hard hat knocking his hat to the mine floor. As he bent down to retrieve his hard hat a rock 1ft. x 1ft. x 1-2in. fell and struck his on the top left side of his head causing a laceration. He received 5 staples to close the laceration.
