Narrative

Employee was standing on a 3' ladder placing the top block on the top of a stopping in the 2-1 crosscut. The ladder failed and the employee fell to the ground while holding the block, resulting in his left third and fourth fingers being caught between the mine floor and the block. Employee suffered a bruise to his fingers.
